Activity Coach-Memory Care  

As an activity coach for our memory care unit you will be asked to plan and coordinate activities that fit the needs of the residents, work alongside the CNAS to provide assistance with transfers as needed. Provide assistance with meals. Coordinate volunteers for the unit. 

 

Works alongside the CNAS under the direction of the Memory Care Nurse  

Conducts interviews with the staff and residents to assist in the planning of meaningful activities 

Provide assistance with meal service including set-up, serving and clean up as needed 

Maintain a balanced program including physical, social, religious, intellectual, and creative arts and crafts.  

Offer activities at hours convenient to the residents (Morning, afternoon, some evenings and holidays.  

Prepare and post monthly activities for Memory Care that reflect the interest of the residents. 

Maintain activity supplies and ensure they are accessible to residents and staff 

Coordinate activity assistance with available volunteers.  

Overview

The Wellness Coach specializes in Exercise Groups and Wellness Programming and is the liaison between Wellness, Rehabilitation Services, and Nursing. 

Responsibilities

Manages and maintains residents’ wellness/restorative nursing programs and reports any changes to the treatment plan.  Conducts exercise groups that meet the MDS criteria for restorative nursing and documents the care provided.  Maintains specialized equipment, such as alarms, cushions, and wheelchairs. Collaborates with interdisciplinary team by planning and anticipating long term and/or short term needs regarding residents’ admission and discharge.

Qualifications

Qualifications Required

  • Successful completion of a Wellness Training and Restorative Nursing Class.

Qualifications Preferred

  • In good standing on the State Nursing Assistant Registry.
  • Experience in a hospital or nursing home setting.
  • CPR certified.
