St. Gertrudes in Shakopee, MN is looking for an on-call Occupational Therapist (OT) to provide as-needed OT treatment to residents. The Occupational Therapist Float is responsible and accountable for residents’ evaluation, ongoing assessment, planning, and implementation of therapy interventions. 

• Develops and revises plan of care based on the residents’ needs and establishes functional goals, outcomes, and discharge plan. 
• Plans for residents’ discharge and anticipates for long term and/or short term residents’ needs. 
• Educates other health care team members, residents, family, and caregivers on information regarding the care plan for the residents.
• Provides treatment to all BHS communities as needed; may be required to travel to multiple sites in a day.

Qualifications Required

• Current state licensure as an Occupational Therapist, in good standing.
• Bachelor’s degree.
