Benedictine is seeking dynamic and compassionate LPN’s who will provide quality patient-centered, evidenced-based care to our greatest assets – our Residents.  

• The LPN is responsible for utilizing the principles of Relationship Based Care while performing duties.
• The LPN is responsible for initial data collection on all residents.
• The LPN participates in implementing person driven care plans as developed by RN.
• Participates in implementing person driven plans of care as developed by RN within established policies and procedures and continually works to meet individual patient/resident needs. 
• Demonstrates knowledge of medications and their correct administration, based on patient/resident clinical condition. Observe, document, and report patient/resident responses to medications. 
• Follows patient/resident plan of care as developed by the Registered Nurse, clearly communicates information and documents patient/resident response to care. 
• Coordinates and direct patient/resident care appropriately. 

• Current State licensure as a Practical Nurse, in good standing.
• Must be able to read, write and communicate in English.
• Clinical experience preferred.
