As a Case Manager-RN you are responsible for facilitating resident admissions and discharges, ensuring seamless transitions through their journey. You will work with families, residents as well as internal/external partners to ensure residents receive high quality care and services.

Responsibilities

  • The RN conducts pre-admission assessment of prospective residents.
  • The RN develops and ensures continuity of dialogue with hospitals/social workers in community and outlying services regarding availability within community.

Qualifications

  • Current state licensure as Registered Nurse
  • Previous admission/discharge experience preferred
  • Be able to read, write, and communicate in English
  • Acceptable driving record that meets Benedictine requirements.

JOB SUMMARY


The primary responsibility of the Case Manager is facilitating resident admissions to and from the community, ensuring
seamless transitions. This position should operate within the scope of practice and enlist assistance and approval when
needed and appropriate.

 


KEY RESULT AREAS/ESSENTIAL FUNCTIONS OF THE JOB


1. Conduct pre-admission assessment of prospective residents for medical needs and documentation of admission
assessment criteria and awareness of continuing criteria for Medicare eligibility.
2. Inform other disciplines and coordinate any equipment required related to admission.
3. Coordinate with new residents, their family and community staff to accomplish a smooth transition to the
community.
4. Develop and ensure continuity of dialogue with hospitals/social workers in community and outlying areas on weekly
basis in regards to availability.
5. Complete admission documentation in Matrix computer system.
6. Complete admission documents with the family or resident.
7. Configure daily case mix and RUGS categories and relay information to nursing schedulers.
8. Communicate payment responsibilities to perspective residents/power of attorney.
9. Determine Medicare A status with MDS nurses.
10. Upon admission, work with Social Services and patient to determine expected discharge plan.
11. Promotes the Benedictine Mission and Core Values of Hospitality, Stewardship, Respect and Justice by bringing the
Mission and Core Values into the day-to-day activities of the company.
12. Performs other duties, tasks and/or projects as assigned.

 


QUALIFICATIONS REQUIRED


• Graduate of an accredited school of nursing
• Current state licensure as a Registered Nurse
• Basic Life Support (BLS) Certification
• Valid driver’s license and a safe driving record
• Flexible schedule to accommodate admissions
