Our Social Services Assistant do an amazing job with assisting in the identification of medically related social services as well as supporting the overall well-being of our residents, including the researching appropriate services.

Responsibilities

  • The Social Services Assistant provides support and advocates for resident and family.
  • The Social Services Assistant participates in discharge planning, development and implementation of social service care plan.
  • The Social Services Assistant assists in vulnerable adult investigations.

Qualifications

  • Experience in admission/discharge procedures preferred
  • Hospital or nursing home experience preferred
  • Able to read, write and communicate in English
