Our Case Manager-Social Workers are one of the first points of contact for our residents and their families during admission into our communities. Our compassionate and caring Case Manager-Social Workers help to make this emotional transition seamless and provides support throughout the process.

Responsibilities

  • The Case Manager-Social Worker manages the admission and discharge process as well as coordinates with internal and external customers.
  • The Case Manager-Social Worker develops and communicates with local hospitals/social workers and outlying areas in regards to availability at the community.
  • The Case Manger-Social Worker works with social services and the resident to determine a discharge plan.

Qualifications

  • Licensed by the Board of Social Work in the State of practice, in good standing
  • Bachelor’s degree from a school accredited in the Council of Social Work education
  • Flexible schedule to accommodate admissions
  • Ability to read, write and communicate in English
